Request new password page is broken

The verification image isn't being displayed which does make it rather hard ;-)

The error is "Failed to load resource: the server responded with a status of 404 (Not Found)"

Hello @ukmoose, I wonder if it was a temporary captcha server outage? Im seeing the image ok now, does it work for you?
